Before you generate the report, build a trip plan to record information about your trip and communicate it to your supervisor and check-in contact. Use our Basic Trip Plan Form (fillable PDF 212KB) or our Risk-Rated Trip Plan Form (fillable PDF 112KB) if your organization doesn’t have its own form. Fill in the form, save it on your device, and proceed to Step 6. Step 6: Submit your TripCheck report and trip planYou’re now ready to finish the TripCheck process by emailing a copy of your work to yourself, your supervisor, and your check-in contact. The email should include: Your TripCheck report The trip plan you completed in Step 5 Upload your completed trip planUpload your completed trip plan I used one of the Road Safety at Work trip plan forms or my organization’s trip plan form, and will upload it here. Accepted file types: pdf, doc, docx, txt, Max. file size: 50 MB.Submit your TripCheck report and your trip planEnter your name and the email address(es) of your supervisor and/or your check-in contact below.
